Critically ill patients are characterized by serious physiological and metabolic derangements, as well as organ dysfunction. These factors alter the pharmacokinetics of drugs administered and have a profound influence on the therapeutic index. Therefore, individual dose-finding is of utmost importance. Consideration should be given to the following: individual patients needs; detailed knowledge of the pharmcokinetic profile of the drugs; estimation of pharmacokinetic parameters such as clearance and volume of distribution, ideal body weight (lean body mass), adjustment of drug elimination according to the Dettli-Normogramm, drug monitoring; computer-based dosing program and contact with the clinical pharmacologist. With the above points in mind, complex pharmacotherapy can be safe, effective and economical.

Kritische Kranke zeigen schwere Veränderungen physiologischer und metabolischer Reaktionen sowie Organdysfunktionen. Diese Faktoren beeinflussen die Pharmakokinetik der Medikamente und führen zu einer Variabilität zwischen Dosis und Wirkung, somit zu toxischen Nebenwirkungen oder zur Unwirksamkeit. Eine Dosisindividualisierung ist unerläßlich. Berücksichtigung der individuellen Patientensituation; detaillierte Kenntnisse der Medikamente anhand von pharmakokinetischen Profilen; Einschätzung pharmakokinetischer Parameter wie Clearance und Verteilungsvolumen, ideales Dosierungsgewicht (lean body mass), Anpassung der Elimination nach dem Dettli-Normogramm; Drug monitoring; computergestützte Dosisprogramme; Kooperation mit dem klinischen Pharmazeuten. So wird die komplexe Pharmakotherapie auf der Intensivstation sicher, effektiv und ökonomisch.
