Summary
Recent reports suggest a poor outcome of orthotopic liver transplantation for patients with hemochromatosis. Nine of the 1016 patients (0.9%) underwent transplantation because of hemochromatosis. Four of nine patients (44.4%) died 10, 19, 95 and 360 days after transplantation. All patients who died, and two patients of the survival group, experienced serious cardiac complications in terms of cardiac congestion (2) and arrhythmias (4), which caused death in three cases. Preoperatively, in the group of the non-survivors, occult coronary artery disease (1), mitral valve insufficiency (1) and beginning dilatative cardiomyopathy (1) were diagnosed. The patients were determined to be eligible for liver transplantation. Patients with hemochromatosis are at high risk of cardiac complications after liver transplantation. Intensive cardiac evaluation should be performed in all potential liver transplant recipients in an effort to exclude cardiac manifestation of hemochromatosis. A liver transplantation is not indicated in the case of evidence of cardiac disease in patients with hemochromatosis. Postoperative management must consider the high cardiac risk in this group.
Zusammenfassung
Die Lebertransplantationen bei hereditärer Hämochromatose zeigen eine hohe postoperative Mortalität. In unserem Zentrum wurden 9 Patienten (0,9%) aufgrund einer Hämochromatose lebertransplantiert. 4 der 9 Patienten (44,4%) verstarben 10, 19, 95 und 360 Tage nach der Transplantation. Bei allen verstorbenen, sowie bei 2 überlebenden Patienten stellten sich frühpostoperativ kardiale Komplikationen i. S. von schwerer Herzinsuffizienz mit Pumpversagen (2), sowie schwerwiegenden Herzrhythmusstörungen (4) ein, die in drei Fällen zum Tode führten. Präoperativ wurden bei drei der verstorbenen Patienten eine KHK (1), geringgradige Mitralinsuffzienz (1) und beginnende dilatativen Kardiomyopathie (1) aufgedeckt. Die übrigen Patienten wurden kardiologisch als unauffällig eingestuft. Die invasive kardiale Diagnostik sollte bei allen Patienten mit Hämochromatose erfolgen. Bei nachweisbarer kardialer Vorschädigung ist die Indikation zur Lebertransplantation als fraglich anzusehen. Die peri- und postoperative Betreuung muß dem erhöhten kardialen Risiko in dieser Gruppe Sorge tragen.
