Zusammenfassung
Das operative Spektrum bei proximalen Humerusfrakturen reicht von minimal invasiven Techniken bis zur primären Prothesenversorgung. Zentrale Bedeutung kommt dabei einer erhaltenen Blutversorgung des Humeruskopfes und knöchernen Heilung der Metaphyse zu, während mäßige Restdeformierungen im glenohumeralen Gelenk funktionell toleriert werden können [25]. Hohe Raten avaskulärer Humeruskopfnekrosen im Zusammenhang mit klassischen Osteosyntheseverfahren [14] haben dazu geführt, dass heute bevorzugt minimal-invasive OP-Techniken zur Anwendung kommen. Während 2- und 3-Fragmentfrakturen meist osteo-synthetisch versorgt werden können, wird bei dislozierten 4-Fragmentfrakturen, zumindest beim älteren Menschen, zunehmend die primäre Prothesenversorgung diskutiert [4, 8].
