Summary

Between January 1993 and September 2000,96 patients (pat.) with malignant tra-cheo-bronchial stenosis were treated with Laser or stent. 50 pat. were treated with Nd: YAG Laser only (1-8times, 112total). Large tumors were treated several times. In all pat. bronchoscopy was performed regularly. Location: trachea 22 pat., main bronchus (br.) 17 pat., both main br.3pat., lobar br.8 pat. (up to 95% stenosis). Primary tumor: 16 BC, 16 metastatic (kidney, colon, sarcoma, hcc, ovar, hypopharynx); 15 trachéal infiltrations (5 thyroid, 41aryngeal, 4 esophageal, 2mediastinal, 1 tracheal); mean follow up 129days (1-1409). 55stents were implanted in 46 pat. (covered or uncovered wallstents): 35 trachéal stents, 20 bronchial stents (26 additional Laser applications); mean follow up 177 days (1-2058). Even almost complete malignant occlusions could be treated successfully.

Zusammenfassung

Von 1/93 bis 9/00 wurden 96 Pat. mit inoperablen malignen tracheobronchialen Stenosen behandelt. Alleinige Nd:YAG-Lasertherapie bei 50 Pat. (Mittel 2.2 Laserungen pro Pat. (1-8), gesamt 112). Große Tumore wurden in mehreren Sitzungen abgetragen, alle Pat. wurden regelmäßig Kontrollen zugeführt. Die Trachea war bei 22 Pat. betroffen, bei 17 Pat. ein Hauptbr., bei 3 beide Hauptbr., bei 8 ein Lappenbr. Der Stenosegrad betrug bis 95%. Primärtumor: 16BC/BC-Rez., 16Metas (Niere, Colon, Sarkom, HCC, Ovar, Hypopharynx), 15 TrachealinFiltrationen von außen (5SD, 4Larynx, 4Ö sophagus, 2 Mediastinal Tu), 1 Tracheal-Ca. Mittl. Nachbeobachtung 129d (l-1409d). 46 Pat. wurden mit 55 covered oder uncovered Wallstents versorgt. 35 Stents wurden in der Trachea, 20 Stents in Haupt- oder Lappenbronchien implantiert, ergänzt durch 26 Laserungen. Mittl. Nachbeobachtung 177d (1-2058d). Endoskopisch wurden auch höchstgradige, inoperable maligne Stenosen rekanalisiert.
