Endoluminale Therapie des Zenker Divertikels. Klinische und radiologische Ergebnisse / Endoscopic Treatment of Zenker’s Diverticulum

Summary

We report on 20 patients (mean age 75.5 years, 12 female, 8 male) with Zenker’s diverticulum treated endoscopically by division of the common wall between esophagus and diverticulum with an endostapler between 5/97 and 9/00. The patients suffered from dysphagia of liquids in 62.5%, dysphagia of solid food in 94.7% and regurgitation in 100%. 18 patients were treated successfully, in 2 cases a switch to open surgery was necessary due to difficult exposure. There was one severe complication of mediastinitis and two aspirations. After a mean follow up of 18 months there were 3 recurrences, in the remaining patients dysphagia of liquids was present in 0%, dysphagia solid food in 8% and regurgitation in 8%. This method offers all advantages of minimally invasive surgery, especially to elderly patients.

Zusammenfassung

Zwischen 5/97 und 9/00 wurden 20 Patienten (Durchschnittsalter 75,5 Jahre, 12 männlich, 8 weiblich) mit Zenker Divertikel durch transorale Spaltung des Septums zwischen Ösophagus und Divertikel mittels Endostapler behandelt. Dysphagie flüssiger Nahrung kam bei 62,5%, fester Nahrung bei 94,7% und Regurgitation bei 100% der Patienten vor. In 18 Fällen war der Primäreingriff erfolgreich, 2 mal musste aufgrund schwieriger Einstellbarkeit abgebrochen werden. An Komplikationen traten eine Mediastinitis und zwei Aspirationspneumonien auf. Nach einem Follow Up von durchschnittlich 18 Monaten gab es 3 Rezidive, bei den übrigen Patienten war Dysphagie flüssiger Nahrung in 0%, fester Nahrung in 8% und Regurgitation in 8% vorhanden. Vor allem alte Patienten profitieren von der minimal invasiven Therapie.
