Proximale Humerusfrakturen — Moglichkeiten und Grenzen der minimal invasiven Versorgung mit dem Titan-Wendel / Proximal Humeral Fractures: Possibilities and Limitations of the Minimally Invasive Treatment with the Helix Wire

Summary

Prospective study to evaluate a new, minimally invasive technique on the proximal humerus of elderly patients. Since 1998 we have used the helix wire in more than 37 patients. The helix wire is an intramedullary device with the following features: Dynamic 3-point stabilization, a coreless, cancellous bone-screw grip and additionally the ability of the lateral soft tissues to act as tensor. 13 patients were followed up after more then one year, according to standardized criteria. 10 consolidations were noted; however, in 3 cases we had to change the stabilization technique because of non-union. Increasingly, we combine this technique with percutaneous screw osteosynthesis. The helix wire as a soft tissue sparing, minimally invasive and technically easy implant is an interesting alternative to existing procedures.

Zusammenfassung

Prospektiv angelegte Studie zur Evaluation einer neuen, minimal invasiven Technik am proximalen Humerus des älteren Patienten. Seit 1998 haben wir 37 Patienten mit einem Titanwendel versorgt. Der Titanwendel ist ein intramedullärer Kraftträger mit folgenden Merkmalen: Dynamische 3-Punkte-Abstützung, kräftige Verankerung im spongiö-sen Humeruskopf und zusätzliche Zuggurtung durch den umgebenden Muskelmantel. 13 Patienten wurden bisher nach einem Verlauf von über 1 Jahr standardisiert nachkontrolliert. 10 mal kam es zur Ausheilung, 3mal mußte wegen einer Non-Union ein Verfahrenswechsel eingeleitet werden. Zunehmend haben wir das Verfahren mit der perkutanen Schrauben-osteosynthese kombiniert. Der Titanwendel stellt als weichteilschonendes, minimal invasives und technisch einfaches Implantat eine interessante Alternative zu den bisherigen Verfahren dar.
