Morbus Crohn-Rezidiv im neoterminalen Ileum nach Ileozökalresektion

Summary

Recurrence of Crohn’s disease requiring repeated surgical therapy represents a therapeutic problem, which is accompanied by extended resections of the gastrointestinal tract. A retrospective analysis of our patients (n = 137) with the diagnosis Crohn’s disease showed 64 who unterwent ileocecal resection and 16 with recurrence of the disease in the neoterminal ileum. Reasons for reoperation were 8 stenoses in the anastomotic area, 5 progressed fistula diseases and 3 combined fistula-stenosis patterns. 7 ileoascendostomas, 7 ileotransversostomas and 3 definitive ileostomas were performed. In spite of standardized regimens for preventive treatment against recurrence (5-ASA monotherapy), a high degree of heterogeneity in preventive drug therapy against postoperative recurrence of Crohn’s disease was evident.

Zusammenfassung

Das Operationspflichtige Morbus-Crohn-Rezidiv nach Ileozökalresektion stellt ein therapeutisches Problem dar, welches häufig Re-Eingriffe mit zum Teil ausgedehnten Resektionen erfordert. Die retrospektive Analyse unseres Patientenguts (n 137) mit Morbus Crohn zeigt 64 Patienten, bei denen eine Ileozökalresektion durchgeführt wurde und 16 Patienten mit Rezidiv im neoterminalen Ileum. Gründe für die ReOperation waren 8 Anastomosenstenosen, 5 Fistelbildung und dreimal die Kombination Stenose/Fistel. Es wurden 7 erneute Ileoascendostomien, 7 Ileotransversostomien und zwei endständige Ileostomien angelegt. Trotz standardisierten Empfehlungen für die medikamentöse Rezidivprophylaxe (Monotherapie mit 5-ASA) fiel eine hohe Heterogenität in der medikamentösen Rezidivprophylaxe auf.
