Schlussfolgerung
Endoskopische Nierenentnahmetechniken haben sich als sichere, schmerzlose und effiziente Alternative im Rahmen der Lebendspendernephrektomie etabliert. Trotz einer vergleichbaren Komplikationsrate von offenen und endoskopischen Techniken kann es, wie in diesem beschriebenen Fall, zu schwerwiegenden und folgeträchtigen Komplikationen kommen. Neben der Einhaltung einer möglichst geringen warmen Ischämiezeit während der Nierenabsetzung stellt die notwendige subtile Präparation der Nierengefäße den schwierigsten und anspruchsvollsten Schritt der Operation dar. Typischerweise linksseitig ist der Nierenhilus von ausgeprägten Lymphgefäßen umgeben, die nicht selten direkt in eine Cisterna chyli oder variabel in einen Plexus lymphaticus münden. Eine Verletzung größerer Lymphgefäße ist zwingend zu verhindern. Iatrogene Verletzungen sollten unmittelbar mittels Titanclips oder, falls möglich, mittels direkter Umstechung mit 4-0 monofilem Faden versorgt werden. Als Prophylaxe der Verletzung größerer Lymphgefäße ist besonders medial der Nierengefäße und anterior der Aorta auf eine dezidierte Koagulation der Lymphgefäße zu achten. Die Verletzung der großlumigen, jedoch sehr dünnwandigen Lymphstammgefäße ist zu verhindern.
