Objectifs
Déterminer les particularités épidémiocliniques, pronostiques et thérapeutiques des cancers du sein triple négatifs (CSTN).
Patientes et méthodes
Étude rétrospective observationnelle portant sur tous les cas de cancer du sein avec récepteurs hormonaux et HER-2 négatifs suivis entre avril 2009 et mars 2012. Étaient étudiées les caractéristiques sociodémographiques des patientes, les particularités diagnostiques des CSTN et les modalités thérapeutiques et évolutives des cas suivis. Les données étaient saisies et analysées à l’aide du logiciel SPSS (Statistical Package for Social Science), version 19.0.
Résultats
Vingt-deux patientes ont été retenues, soit 33,8 % de l’ensemble des cancers du sein ou 5,7 % des pathologies mammaires enregistrées durant la période d’étude. L’âge moyen des patientes était de 45,4 ans. La majorité (63,6 %) avait moins de 50 ans; 59,1 % étaient non ménopausées. La gestité moyenne était de 4 et la parité moyenne de 3,6. Le délai moyen de consultation était de 11,1 mois. La découverte d’une masse mammaire était le motif de consultation dans la quasi-totalité des cas (95,5 %). Le cancer était localement avancé dans 86,3 % des cas. Une atteinte ganglionnaire axillaire était présente chez 68,1 % des patientes. Dans 27,3 %, il y avait déjà des localisations secondaires au moment du diagnostic. Le carcinome canalaire infiltrant représentait 86,4 % des types histologiques. Nous avions trouvé 68,2 % de tumeurs de grade 3. Pour 59 % des patientes, le traitement initial était une chimiothérapie néoadjuvante avec une réponse dans 61 % des cas. La chirurgie dans la totalité des cas était une mastectomie selon la technique de Patey modifiée suivie d’un curage axillaire. Trois patientes ont bénéficié d’une radiothérapie. Le délai moyen de récidive était de 16,9 mois. Cette récidive était observée dans 27 % des cas. La médiane de survie globale était de 25 mois (21,1–28,8, IC 95 %) et la médiane de survie pour les patientes M+ de 16,5 mois. Le taux de survie à cinq ans était de 41 %.
Conclusion
Cette série met en évidence une fréquence élevée des CSTN. Ces tumeurs sont très agressives avec un pronostic très péjoratif. Au vu de ces résultats, une collaboration avec des équipes de chercheurs des pays développés est nécessaire pour mieux cerner les CSTN en Afrique afin d’améliorer le pronostic.

Objectives
To determine the epidemiological and clinical characteristics, prognosis, and therapeutic modalities of triple negative breast cancer.
Patients and methods
It is a retrospective observational study of all cases of breast cancer with hormone receptors and HER-2 negative followed between April 2009 and March 2012. The following were studied: the sociodemographic characteristics of patients, the diagnostic features of triple negative breast cancer, and therapeutic modalities and outcome of followed cases. Data were entered and analyzed using SPSS software, version 19.0.
Results
Twenty-two patients were included. The rate was 33.8% of all breast cancers and 5.7% of breast pathologies recorded during the study period. The average age of patients was 45.4 years. The majority (63.6%) had less than 50 years. A proportion of 59.1% were premenopausal. The mean gravidity was 4 and mean parity 3.6. The mean delay for consultation was 11.1 months. The discovery of a breast mass was the reason for consultation in almost all cases (95.5%). Cancer was locally advanced in 86.3% of cases. Axillary lymph node involvement was present in 68.1% of patients. In 27.3% of cases, there were already secondary locations at the time of diagnosis. Infiltrating ductal carcinoma accounted for 86.4% of histological types. We found 68.2% of grade 3 tumors. In 59% of patients, the initial treatment was neoadjuvant chemotherapy. A clinical response was obtained in 61% of cases. Surgery in all cases was a mastectomy according to Patey-modified technique followed by axillary dissection. Three patients received radiotherapy. The mean time to recurrence was 16.9 months. This recurrence was observed in 27% of cases. The median overall survival was 25 months (from 21.1 to 28.8, CI 95%), and the median survival for metastatic patients was 16.5 months. The survival rate at 5 years was 41%.
Conclusion
This series shows a high frequency of triple negative breast cancer. These tumors are very aggressive with a very poor prognosis. In view of these results, collaboration with research teams in developed countries is needed to better identify triple negative breast cancer in Africa to improve prognosis.
