Samenvatting
Ernstige brandwonden als gevolg van hoogvoltage elektriciteitsletsel komen in Nederland minder frequent voor. Doordat er veel warmte wordt geproduceerd tussen de in- en uittredeplaats van de stroom, kan er naast uitwendig zichtbare brandwonden ook ernstige, niet direct zichtbare inwendige schade ontstaan, die potentieel letaal is. Om de mortaliteit en morbiditeit van patiënten met hoogvoltage elektriciteitsverbrandingen te verlagen, zijn adequate vochtresuscitatie, het bedacht zijn op acuut nierfalen, het tijdig verrichten van necrotectomieën, fasciotomieën en amputaties van essentieel belang. Aan de hand van twee ziektegeschiedenissen worden de opvang en zorg van hoogvoltage elektriciteitsletsel geïllustreerd.
Abstract
Burns caused by high-voltage are less common among all severe burns observed in the Netherlands. The highvoltage burns require an extensive Advanced Trauma and Life Support (ATLS). Two cases of high-voltage electric burns are described in order to understand the proper approach to treat a high-electric injury. Highvoltage electric burns are associated with high mortality, morbidity and high risk of amputations. Timely debridements, fasciotomy and escharotomy and adequate fluid approach are required to reduce the mortality and morbidity.
