Background
Patients often see different general practitioners if they require palliative care in the weekend. In order to improve the continuity of personnel, the Nijmegen General Practitioner Cooperative set up a team of specialist palliative care GPs, so that one GP provided palliative care services over the whole weekend. We investigated patients’ opinions about this type of care compared with regular palliative care
Methods
A survey was held among palliative patients who had received, on alternate weekends, care provided by regular GPs and by a palliative care GP for 37 weekends.
Results
Patients awarded both groups of GPs means scores of 8 (range 1, very bad, to 10, very good) for medical and communicative aspects. The palliative care GPs had significantly higher mean scores for expertise (8.9 versus 8.0 for regular GPs) and trust (9.1 versus 8.4 for regular GPs). They also spent more time on home visits (longer than 30 minutes 48% versus 14% for regular GPs) and telephoned the patient more often after a home visit (37% versus 28% for regular GPs). This was appreciated by all patients, who gave palliative care GPs a total score of 9.1 and regular GPs a total score of 8.4.
Conclusions
Patients were very statisfied with the palliative care provided by palliative care GPs at the GP cooperative, but they were also satisifed with the current palliative services provided by the cooperative. The experiment with the palliative care GPs has been stopped because the benefits were outweighed by the large investment in personnel. The basic principle remains that 24/7 palliative care should preferently given by a patient’s own GP.

Doel
Patiënten die palliatieve zorg behoeven krijgen in het weekend vaak te maken met verschillende dienstdoende huisartsen. Om de persoonlijke continuïteit te bevorderen heeft de Coöperatieve Huisartsendienst Nijmegen (CHN) een pool palliatievezorghuisartsen (PPZ) geformeerd, waarvan om het weekend één huisarts een heel weekend dienst deed voor palliatieve patiënten van de huisartsenpost. We onderzochten de ervaringen van deze patiëntengroep met de huidige zorg op de huisartsenpost en gingen na of de inzet van een PPZ-huisarts tot positievere ervaringen bij de patiënt heeft geleid.
Methode
Vragenlijstonderzoek onder palliatieve patiënten gedurende 37 weekenden. Hierbij gaf de niet-PPZ-arts om het weekend standaard (niet-PPZ) zorg of gaf een PPZ-arts de zorg.
Resultaten
Patiënten gaven op medische en communicatieve aspecten zowel niet-PPZ-artsen als PPZ-artsen gemiddelde rapportcijfers van rond de 8. De PPZ-artsen scoorden significant hoger op deskundigheid (8,9 versus niet-PPZ-arts 8,0) en vertrouwen (9,1 versus niet PPZ-arts 8,4). PPZ-artsen namen significant meer tijd voor een visite (48% langer dan een half uur versus niet- PPZ-arts 14%) en namen vaker achteraf telefonisch contact op met de patiënt (37% versus niet-PPZ-arts 28%). Patiënten hebben dit in alle gevallen gewaardeerd. Patiënten gaven de PPZ-arts het eindcijfer 9,1 en de niet-PPZ-arts een 8,4.
Conclusie
Patiënten zijn zeer tevreden over de palliatieve zorg door de PPZ-huisarts op de huisartsenpost. Ze waren echter ook al tevreden over de huidige palliatieve zorg op de huisartsenpost. Het experiment met de PPZ-arts is gestopt omdat de (personele) investering volgens de betrokken huisartsen niet opwoog tegen de opbrengst. Het basisprincipe blijft 24/7 palliatieve zorg bij voorkeur door de eigen huisarts van de patiënt.
