Ergebnisse der schaftfreien Implantate bei posttraumatischer Arthrose aus dem Deutschen Schulterendoprothesenregister

Outcomes of the stemless prosthesis for posttraumatic sequelae from the German Shoulder Replacement Registry

Zusammenfassung

Hintergrund

Die operative Versorgung posttraumatischer Omarthrosen stellt eine besondere Herausforderung dar, da es sich häufig um jüngere Patienten mit deutlicher Verformung des proximalen Humerus handelt.

Ziel der Arbeit

In der vorliegenden Studie werden die mittelfristigen Ergebnisse nach Versorgung posttraumatischer Omarthrosen mittels schaftfreier Schulterprothesen dargestellt.

Material und Methoden

Im Schulterprothesenregister der Deutschen Vereinigung für Schulter- und Ellenbogenchirurgie e. V. (DVSE) sind insgesamt 1052 vollständig dokumentierte primärimplantierte Prothesen erfasst, davon wurden 46 Schultern mittels schaftfreier Prothese bei posttraumatischer Omarthrose mit einem Mindestnachuntersuchungszeitraum von 2 Jahren versorgt. Das durchschnittliche Follow-up betrug 4,1 (3,7–7,6) Jahre.

Ergebnisse

Alle Bewegungsrichtungen verbesserten sich signifikant. Der absolute Constant-Score verbesserte sich insgesamt von 29,9 auf 65 Punkte. Subjektiv waren 25 (54 %) der Patienten sehr zufrieden, 14 (30 %) zufrieden. 4 (9 %) waren unzufrieden und 1 Patient enttäuscht. 2 Patienten machten keine Angaben. 22 Patienten wiesen eine A1- (48 %), 10 eine A2- (22 %), 4 eine B1- und 2 eine B2-Pfanne nach Walch auf. Bei 8 Patienten lagen keine Angaben zur Pfannenveränderung vor. 10 Patienten wurden mit einem Glenoidersatz versorgt. Bei 9 Patienten fand eine Rekonstruktion der Supraspinatussehne statt, in 5 Fällen wurde die Refixation der Subscapularissehne als kritisch beurteilt. Der Humeruskopf war bei 40 Patienten (87 %) radiologisch zentriert. Einmal fand sich ein Lysesaum um die Schaftkomponente. Einmal fand ein Wechsel auf eine inverse Prothese statt.

Schlussfolgerung

Die Versorgung der posttraumatischen Omarthrose mit einer schaftlosen Prothese als Alternative zur inversen Prothese stellt im mittelfristigen Verlauf eine gute Therapieoption gerade bei jüngeren Patienten dar. 84 % der Patienten waren mit der Versorgung zufrieden.

Abstract

Background

The surgical treatment of posttraumatic sequelae presents a particular challenge, since it often affects younger patients with marked deformation of the proximal humerus.

Aim of the work

In the present study, the medium-term results after stemless shoulder arthroplasty for patients with posttraumatic sequelae are demonstrated.

Materials and methods

The shoulder prosthesis register of DVSE contains a total of 1052 fully documented primary implanted prostheses, of which 46 shoulders were treated with a stemless prosthesis in post-traumatic arthritis with a minimum follow-up of 2 years. The average follow-up was 4.1 (3.7–7.6) years.

Results

All directions of motion improved significantly. The absolute constant score improved from 29.9 to 65 points. Subjectively, 25 (54%) of the patients were very satisfied, 14 (30%) were satisfied, 4 (9%) were dissatisfied and 1 patient disappointed, while 2 patients gave no information. A total of 22 patients had an A1 (48%), 10 an A2 (22%), 4 × B1 and 2 × B2 glenoid according to Walch, and in 8 × no information was given. Ten patients were treated with a glenoid replacement. There were 9 × reconstructions of the supraspinatus tendon, in 5 cases the refixation of the subscapularis tendon was assessed as critical. The humeral head was radiologically centered in 40 patients (87%). One patient received a change to a reverse prosthesis.

Conclusion

The treatment of posttraumatic osteoarthritis with a stemless prosthesis as an alternative to the reversed total shoulder replacement represents a good treatment option in the medium term, especially in younger patients. In all, 84% of the patients were satisfied with this treatment.
