Zusammenfassung
Ein interdisziplinäres Leitlinienteam mit Experten aus dem Bereich der Gynäkologie und Geburtshilfe, der Diabetologie, der inneren Medizin, Kinderheilkunde und Hebammenwissenschaften aus Deutschland, Österreich und der Schweiz verfasste eine neue S3-Leitlinie zur Diagnostik, Therapie und Nachsorge bei Gestationsdiabetes, die im Frühjahr 2018 bei der AWMF (Arbeitsgemeinschaft der Wissenschaftlichen Medizinischen Fachgesellschaften e.V.) unter der Nr. 057/008 veröffentlicht wurde. Ziel der Leitlinie ist, eine Orientierung für ein individuell angepasstes Management je nach Ausprägung der Glukoseintoleranz und sonstigen vorbestehenden maternen Risiken oder geburtshilflichen Komplikationen zu geben, immer unter der Berücksichtigung einer möglichst geringen Belastung der Schwangeren. Der vorliegende Beitrag konzentriert sich auf einige klinisch relevanten Aspekte der Leitlinie, in denen sich durch Einbeziehung neuer Studien Veränderungen im Management ergaben. Das betrifft vornehmlich das Screening in der Frühschwangerschaft bei vorhandenem Diabetesrisiko, das GDM-Screening (GDM: Gestationsdiabetes mellitus) im 3. Trimenon und die Validität des 50 g-Tests, das Abwägen der therapeutischen Intervention, die Gabe oraler Antidiabetika, die Geburtsplanung und das kindliche Langzeitoutcome nach Schwangerschaften mit GDM.
Abstract
An interdisciplinary guideline team made up of experts in gynecology and obstetrics, diabetology, internal medicine, pediatrics and midwifery from Germany, Austria, and Switzerland have drawn up a new S3 guideline on the diagnosis, treatment, and follow-up of gestational diabetes. The guideline was published in spring 2018 by the German Association of the Scientific Medical Societies (Arbeitsgemeinschaft der Wissenschaftlichen Medizinischen Fachgesellschaften e.V., AWMF) under the number 057/008. The aim of the guideline is to provide guidance for individually tailored management according to the degree of glucose intolerance, as well as other pre-existing maternal risks or obstetric complications, while kee** the burden on the patient as low as possible. This article concentrates on a number of clinically relevant aspects of the guideline, in which the inclusion of new studies has led to changes in management. This applies in particular to screening in early pregnancy in the case of a risk for diabetes, gestational diabetes mellitus (GDM) screening in the third trimester, the validity of the 50‑g test, assessing the therapeutic intervention, the administration of oral antidiabetic drugs, delivery planning, and the long-term outcome in children following GDM pregnancies.
