Zusammenfassung
Hintergrund
Neben erheblichen körperlichen Einschränkungen leiden Menschen mit interstitiellen Lungenerkrankungen (ILD) häufig unter einer verminderten Lebensqualität und psychischen Begleitphänomenen.
Ziel der Arbeit
Im klinischen Alltag wird psychischen Begleiterscheinungen interstitieller Lungenerkrankungen (z. B. Depressivität und Angstsymptomatik) häufig noch wenig Aufmerksamkeit geschenkt, obgleich die Forschung in diesem Bereich in den letzten 10 Jahren kontinuierlich zugenommen hat. Dieser Beitrag soll einen Überblick über die Häufigkeit, das Erscheinungsbild und die Zusammenhänge dieser psychischen Begleiterscheinungen mit dem Krankheitsgeschehen geben.
Material und Methoden
Es wurde eine Literaturübersicht erstellt, indem relevante Studien zu Depressivität und Angstsymptomatik bei interstitiellen Lungenerkrankungen durch eine umfangreiche Recherche in der elektronischen Datenbank PubMed identifiziert wurden. Die Suche war auf Beiträge in englischer Sprache aus dem Zeitraum 2008 bis 2023 beschränkt.
Ergebnisse
Die Prävalenz von psychischen Begleiterscheinungen bei ILD-Patienten liegt zwischen 14,3 und 31,7 % für moderate bis schwere Depressivität und zwischen 17,6 und 31 % für moderate bis schwere Angstsymptomatik. Diese können eine komplexe Rolle für das Krankheitsgeschehen spielen und sich negativ auf die Lebensqualität auswirken.
Diskussion
Die Berücksichtigung von Depressivität und Angstsymptomatik bei der Behandlung von interstitiellen Lungenerkrankungen ist von hoher klinischer Relevanz. Die komplexen Wirkzusammenhänge erfordern weitere Forschungsbemühungen.
Abstract
Background
In addition to significant physical limitations, individuals with interstitial lung diseases (ILD) often experience reduced quality of life and psychological comorbidities.
Objective
In the routine clinical practice psychological comorbidities, such as depression and anxiety in patients with ILD receive less attention, despite an increase in research over the past decade. This article aims to provide an overview of the prevalence, manifestation and relationship of these psychological conditions with the disease process.
Material and methods
A literature review was conducted to identify studies on depression and anxiety in ILD through a comprehensive search of the PubMed electronic database. The search was limited to articles published in English from 2008 to 2023.
Results
The prevalence of psychological comorbidities among ILD patients ranges from 14.3% to 31.7% for moderate to severe symptoms of depression and from 17.6% to 31% for moderate to severe symptoms of anxiety. These conditions can play a complex role in the course of the disease and negatively impact the quality of life.
Conclusion
The consideration of anxiety and depression in the treatment of ILD can be clinically beneficial. The complex interactions involved suggest that further research is needed.

Als „psychische Komorbidität“ bezeichnet man das gleichzeitige Vorliegen einer psychischen Störung neben einer körperlichen Erkrankung. Es liegt also parallel eine psychische Diagnose (nach DSM [Diagnostic and Statistical Manual of Mental Disorders] oder ICD [International Statistical Classification of Diseases and Related Health Problems]) vor. Im vorliegenden Review beziehen wir in unsere Betrachtung auch subklinische psychische Syndrome mit ein. Deswegen nutzen wir an vielen Stellen den Begriff „psychische Begleitsymptomatik“.
