Zusammenfassung
Hintergrund
Das Basalzellkarzinom ist der häufigste Tumor des hellhäutigen Menschen.
Fragestellung
Es erfolgt die Darstellung von Pathogenese, Epidemiologie, Diagnostik, Therapie und Nachsorge des Basalzellkarzinoms.
Material und Methoden
Dazu wurden Leitlinien und publizierte Studien ausgewertet.
Ergebnisse
Pathogenetisch liegt dem Basalzellkarzinom eine Aberration des Hedgehog-Signalwegs mit konstitutioneller Aktivierung zugrunde. Die Inzidenz in Deutschland liegt bei mindestens 200/100.000 Einwohnern pro Jahr. Frühe Läsionen können klinisch eine diagnostische Herausforderung darstellen, hier können nichtinvasive Diagnoseverfahren wie Auflichtmikroskopie, optische Kohärenztomographie und konfokale Laserscanmikroskopie hilfreich sein. Die klinische Verdachtsdiagnose wird i. d. R. mittels Gewebeuntersuchung histologisch bestätigt. Die Standardtherapie besteht in einer histologisch gesicherten kompletten Resektion. Therapeutisch können insbesondere multiple Basalzellkarzinome (wie beim Gorlin-Goltz-Syndrom) oder lokal fortgeschrittene bzw. selten auch metastasierende Basalzellkarzinome ein Problem darstellen. Bei oberflächlichen Basalzellkarzinomen können nichtoperative Verfahren wie die photodynamische Therapie oder topische Medikamente erwogen werden. Bei lokal inoperablen oder metastasierenden Basalzellkarzinomen sollten in einer interdisziplinären Tumorkonferenz Therapieempfehlungen ausgesprochen werden, zu den Optionen zählen hier eine Strahlentherapie oder eine Systemtherapie mit Hedgehog- oder PD-1-Inhibitoren.
Schlussfolgerungen
Mit dem Basalzellkarzinom werden aufgrund seiner Häufigkeit viele Fachdisziplinen konfrontiert, die mit dem aktuellen Diagnose- und Therapieoptionen vertraut sein müssen.
Abstract
Background
Basal cell carcinoma represents the most common tumor in persons with fair skin.
Objectives
Summary of pathogenesis, epidemiology, diagnosis, therapy and aftercare of basal cell carcinoma.
Materials and methods
Review of current guidelines and published studies.
Results
The pathogenesis of basal cell carcinoma relies on an aberration of the hedgehog signalling pathway, leading to a constitutive activation of the pathway. The incidence rate in Germany is about 200 per 100,000 inhabitants per year. The clinical diagnosis of early lesions may be difficult. Noninvasive diagnostic tools such as optical coherence tomography and confocal laser scanning microscopy might be helpful in such situations. The clinical diagnosis is usually confirmed by biopsy and histological examination. The standard therapy is histologically controlled complete excision. In particular multiple basal cell carcinomas (such as in Gorlin–Goltz syndrome) and locally advanced or rarely metastatic basal cell carcinomas can represent therapeutic problems. In superficial basal cell carcinomas, nonsurgical therapies such as photodynamic therapy or topical medications can be considered. In case of locally advanced or metastatic basal cell carcinoma, therapeutic options such as radiation therapy for inoperable disease or systemic therapy for inoperable or metastastic tumor with a hedgehog inhibitor or PD‑1 antibody should be discussed by an interdisciplinary tumor board.
Conclusions
Due to its frequency, many disciplines have patients presenting with basal cell carcinoma, which requires knowledge on the state of the art diagnostic and therapeutic options.
