Zusammenfassung
Das Lernziel in den Kursen zum Erlernen manualmedizinischer Kompetenzen ist die Vermittlung situationsgerechter und sicherer Anwendung manualmedizinischer Techniken sowie einer funktionellen Denkweise bezogen auf die jeweilige Region im Bewegungssystem. In diesem Beitrag werden am Beispiel einer Innenrotationseinschränkung des Hüftgelenks manualmedizinische Befund- und Therapieprinzipien beschrieben. Die zuvor benannte behandlungsbedürftige Dysfunktion entspricht oft nicht der vom Patienten angegebenen Schmerzregion (in diesem Fall die untere Lendenwirbelsäule, LWS). Beginnend mit einer Screeninguntersuchung der betroffenen Region werden Auffälligkeiten in spezifischeren Untersuchungsschritten differenziert (Innenrotationseinschränkung des Hüftgelenks). Es folgt eine passive Untersuchung des Hüftgelenks und die Differenzierung der bewegungslimitierenden Strukturen (Gelenkkapsel, Muskel). Möglich ist am Hüftgelenk u. a. ein Bewegungslimit aufgrund von Osteophyten. Dieser Untersuchungsbefund ist über einen Schmerzprovokationstest und ein am Bewegungsende hartes Endgefühl gegenüber den manualmedizinisch beeinflussbaren Strukturen differenzierbar. In diesem Beitrag wird der Fokus auf die Differenzierung manualmedizinisch beeinflussbarer Strukturen (reversible Dysfunktion) gelegt. Die bewegungslimitierende Struktur wird entsprechend manualmedizinischer Grundsätze behandelt. Exemplarisch für eine muskuläre Einschränkung der Innenrotation werden die Testung und Dehnung des M. piriformis angeführt. Für die Erweiterung der Innenrotation bei kapsulärer Restriktion wird eine anguläre Gelenktechnik beschrieben. Ziel der Techniken ist eine Verbesserung der Hüftgelenkbeweglichkeit, um angrenzende Gelenkregionen zu entlasten(in diesem Fall die untere LWS).
Abstract
The learning objective in the courses on acquiring manual medical skills is to teach the safe application of manual medical techniques appropriate to the situation, as well as a functional mindset related to the respective region in the musculoskeletal system. This article describes manual medical principles of diagnosis and treatment using restricted internal rotation of the hip joint as an example. The aforementioned dysfunction requiring treatment often does not correspond to the region of pain indicated by the patient (in this case the lower lumbar spine, LS). Starting with a screening examination of the affected region, abnormalities are differentiated in more specific examination steps (restricted internal rotation of the hip joint). This is followed by a passive examination of the hip joint and differentiation of the structures limiting movement (joint capsule, muscle). Possible findings at the hip joint include a limit to movement due to osteophytes. This finding can be differentiated from the structures that can be influenced by manual medicine by means of a pain provocation test and the sensation of a hard end at the end of movement. This article focuses on differentiating structures that can be influenced by manual medicine (reversible dysfunction). The movement-limiting structure is treated according to manual medical principles. Testing and stretching of the piriformis muscle are given as examples of muscular limitation of internal rotation. To extend internal rotation in the presence of capsular restriction, an angular joint technique is described. The aim of the techniques is to improve hip joint mobility in order to relieve adjacent joint regions (in this case the LS).
