Zusammenfassung
Unterbauchschmerzen sind ein häufiges Symptom in der Schwangerschaft und können zahlreiche Ursachen haben, von der physiologischen Anpassung bis hin zum akuten Notfall. Das Spektrum möglicher Differenzialdiagnosen hängt vom Zeitpunkt des Auftretens in der Schwangerschaft ab. Die primäre Herausforderung besteht darin, ernsthafte oder sogar lebensbedrohliche Diagnosen, wie zum Beispiel eine Extrauteringravidität oder eine vorzeitige Plazentalösung, zu unterscheiden von solchen, welche die Mutter und/oder den Feten nicht unmittelbar bedrohen. Als bildgebende Diagnostik sind die Ultrasonographie und die Magnetresonanztomographie im gesamten Verlauf der Schwangerschaft unbedenklich einsetzbar. Wir möchten im folgenden Artikel einen Überblick über die möglichen Differenzialdiagnosen in früher und später Schwangerschaft geben und einen zielführenden diagnostischen Pfad aufzeigen.
Abstract
Lower abdominal pain is a frequent symptom during pregnancy and can have numerous causes, ranging from physiological adaptation to an acute emergency. The spectrum of possible differential diagnoses depends on the time of occurrence during pregnancy. The primary challenge is to distinguish serious or even life-threatening diagnoses, such as an ectopic pregnancy or premature placental abruption from those that do not directly threaten the mother and/or the fetus. Ultrasonography and magnetic resonance imaging can be used without hesitation as imaging diagnostics throughout the course of pregnancy. This article provides an overview of the possible differential diagnoses in early and late pregnancy and presents a target-oriented diagnostic pathway.
