Samenvatting
Een 36-jarige bodybuilder werd verwezen naar de afdeling Spoedeisende Hulp (SEH) vanwege een ruptuur van de m. pectoralis ontstaan tijdens bankdrukken onder sintromgebruik. De voorgeschiedenis vermeldt een virale myocarditis en verminderde hartfunctie. Bij lichamelijk onderzoek was er een forse zwelling ter plaatse van de m. pectoralis rechts. Boven de rechteroksel bevond zich een hematoom. De INR bedroeg 3,5. Echograne en MRI toonden een hematoom en ruptuur van de m. pectoralis majorrechts op de tendinomusculeuze overgang. De behandelingbestond uit operatief herstel van de m. pectoralis major, gevolgd door het dragen van een sling gedurende zes weken. Ruptuur van de m. pectoralis major is een relatief zeld-zaam trauma. Met behulp van anamnese (traumamecha-nisme) en lichamelijk onderzoek kan vaak de diagnose worden gesteld. Aanvullend beeldvormend onderzoek is nuttig om de plaats van de ruptuur te bepalen. Vroege chirurgische interventie geeft betere resultaten dan con-servatieve behandeling.
Abstract
A 36-year-old bodybuilder was referred to the Emergency Department because of a rupture of the right pectoralis major tendon following bench pressing while using acenocoumarol. Medical history involves a viral myocarditis and suboptimal function of the heart. On physical examination he was found to have a signifi cant hematoma superior to the right axilla. The INR was 3,5. Ultrasound examination and MRI revealed a hematoma and rupture of the pectoralis muscle at the musculotendinous junction. Treatment consisted of operative repair of the tendon, followed by wearing a sling for six weeks. Rupture of the pectoralis tendon is an uncommon injury. Diagnosis can usually be made based on patient’s history (trauma mechanism) and physical examination. However, ultrasound or MRI is useful to investigate the site of rupture. Early surgical intervention shows better results than conservative treatment.
