Primary hepatic malignancy. Efficacy of partial and total hepatectomy with orthotopic liver transplantation

Zusammenfassung

Von 1977 bis 1986 wurden von 172 primären Malignomen der Leber 90 kurativ operiert: 64 Resektionen und 26 Lebertransplantationen, dabei 31 bzw. 65% bei Leberzirrhose. Große Resektionen bei Malignomen in Zirrhose werden wegen der zu erwartenden hohen Letalität (>50%) seit 1982 nicht mehr durchgeführt, über den gesamten Zeitraum verstarben 25% der Resezierten und 34% der Transplantierten. Das Schicksal nach der unmittelbaren perioperativen Phase wird in beiden Gruppen durch Tumorrezidive bestimmt, wobei die mediane Lebenserwartung mit 18,4 Monaten nach R-0-Resektionen und 18,6 Monaten nach LTX nahezu ident ist.

Summary

From 1977 to 1986 curative operations were performed in 90 out of 172 cases of primary hepatic malignancies, 31 resp. 65% of which were combined with cirrhosis (64 liver resections, 26 LTX). From 1982 on extended resections have no longer been performed in cirrhotic patients because of high perioperative risk (more than 50%). During the whole period 25% died after liver resection and 34% after TX perioperatively. The fate of survivors in both groups is determined by tumour recurrence, almost identical median survival times of 18.4 months after R-0-resections and 18.6 months after LTX can be expected.
