Zusammenfassung
Grundlagen
Die Gefahr von klappenbezogenen Komplikationen wie Thromboembolien und die Notwendigkeit der Antikoagulation bei mechanischen Klappenprothesen führten zur Entwicklung neuer operativer Techniken.
Methodik
Beim pulmonalen Autograft wird die erkrankte Aortenklappe durch die eigene Pulmonalklappe ersetzt, die Pulmonalklappe muß jedoch mit einem körperfremden pulmonalen Homograft ersetzt werden. In den letzten 3 Jahren wurde bei 36 Patienten (Alter 24,5±11 Jahre; 6 bis 50 Jahre) wegen Aortenklappenerkrankungen (29 Aorteninsuffizienzen, 5 kombinierte Aortenvitien, 2 Aortenstenosen) eine pulmonale Autograft-Operation ohne perioperative Letalität durchgeführt.
Ergebnisse
Präoperativ war die durchschnittliche NYHA-Klassifikation 2.6±0,2. Innerhalb eines Jahres mußten 2 Patienten (12 und 27 Jahre alt) wegen einer signifikanten AI reoperiert werden (Perikardpatchverschluß wegen Klappenperforation). Die präoperative mittlere AI von 3,4±0,5 verringerte sich auf postoperativ 0,5±0,45. Die Patienten zeigten eine signifikante Verkleinerung der mittleren Ventrikeldurchmesser (präoperativ: ESD=51,7±7,8 mm, EDD=68,3±8,7 mm; postoperativ: ESD=38,5±7,1 mm, EDD=52,8±6,9 mm; p=0,002) und postoperativ eine gute Ventrikelfunktion (FS=0,27±0,09). Die präoperativen Gradienten über der Aortenklappe normalisierten sich auf 7,7±3,5 mm Hg postoperativ. Nach durchschnittlich 11 Monaten befanden sich 22 von 23 bisher nachuntersuchten Patienten in NYHA-Klasse I. Ein Patient mit rheumatischer Genese hatte eine mittelgradige AI; dieser Patient ist in NYHA-Klasse II. Die durchschnittlichen Ventrikeldimensionen (ESD=35,0±4,5 mm, EDD=51,2±4,0 mm) die Ventrikelfunktion (FS=0,32±0,06) und Dopplergradienten (5,8±2,9 mm Hg) sind im Normbereich. Die mittlere AI=0,6±0,5 zeigt keine Veränderung zur unmittelbar postoperativen Untersuchung.
Schlußfolgerungen
Die durch Aortenklappenersatz mit pulmonalem Autograft erzielten hämodynamischen Ergebnisse und die Lebensqualität der Patienten sind denen nach Ersatz mit mechanischen Prothesen überlegen. Probleme sehen wir in der komplexen Operation und der noch nicht bestätigten Haltbarkeit des pulmonalen Homografts.

Background
Valve related complications as thromboembolic events and the neccessity of anticoagulation in patients with mechanical prosthesis lead to new operative techniques in replacement of the aortic valve.
Methods
In the Pulmonary Autograft Procedure, the aortic valve is replaced with the patients pulmonic valve; the pulmonic valve is replaced with a pulmonary homograft. In the last 3 years, 36 patients (age: 24.5±11 years; 6 to 50 years) underwent pulmonary autograft valve replacement (29 aortic insufficiency, 5 combined aortic disease, 2 aortic stenosis) without any perioperative mortality.
Results
The preoperative NYHA classification was 2.6±0.2. Within the first year, 2 patients (age: 12 and 27 years) were reoperated because of severe AI (2 pericardial patches). The mean grade of AI decreased from preoperative 3.4±0.5 to 0.5±0.5 postoperatively. Patients show significant decrease in ventricular dimensions (preoperatively: ESD=51.7±7.8 mm, EDD=68.3 ±8.7 mm; postoperatively: ESD=38.5±7.1 mm, EDD=52.8 ±6.9 mm; p<0.002) and good ventricular function (FS=0.27 ±0.09) postoperatively. Doppler gradients are normal postoperatively (7.7±3.5 mm Hg).
After a mean follow up of 11 months, 22 of 23 patients are in NYHA class I. One patient with rheumatic heart disease has moderate AI, he is in NYHA class II. The mean ventricular dimensions (ESD=35.0±4.5 mm, EDD=51.2±4.0 mm), mean ventricular function (FS=0.32±0.06) and mean Doppler gradients (5.8±2.9 mm Hg) are normal. There was no change in AI (0.6 ±0.5) since discharge.
Conclusions
In patients after Pulmonary Autograft Procedure the hemodynamic results and the life quality are better than in patients with mechanical prosthesis. Problems are the more complicated operation and the question about long-term viability of the pulmonary homograft.
