Zusammenfassung
Grundlagen: Die schnelle Restitution ermöglicht den raschen Schluß von Epitheldefekten durch Enterozytenmigration. Wir untersuchten den Einfluß extrazellulärer Matrixproteine auf die Restitution der säuregeschädigten Mukosa des Kaninchenduodenums in vitro.
Methodik: Schleimhautpräparationen des Kaninchenduodenums wurden in Ussing-Kammern eingebracht und nach einer 30minütigen Äquilibrierungsphase durch 10 min luminale Exposition mit 10 mM HCl geschädigt. Danach wurden die Gewebe 3 Stunden mit Puffer (Kontrollen) oder Puffer, der auf der luminalen Seite die Testsubstanzen enthielt, inkubiert (n=6 pro Gruppe). Potentialdifferenz und Kurzschlußstrom wurden gemessen, der Widerstand (R) nach dem Ohmschen Gesetz berechnet. Nach Versuchsende wurden HE-gefärbte histologische Schnitte hergestellt, das Ausmaß der Schädigung mit einer speziellen Morphometrieanlage gemessen.
Ergebnisse: Nach Schädigungsende waren 58±4%, nach 1 und 3 h 40±4% bzw. 36±2% geschädigt (Kontrollen, n=6). Nach HCl-Exposition lösten sich die geschädigten Enterozyten der oberen 2 Zottendrittel ab, so daß luminal denudierte Stummelzotten resultierten, welche 3 h nach Schädigung teilweise von flachen Enterozyten bedeckt waren. Luminales Rinderserumalbumin (38±5%), Antikörper gegen Kollagen IV (35±7%) und Fibronektin (38±4%) sowie jeweils 25, 50 und 100 µg/ml Kollagen IV (34±3, 35±2 bzw. 33±4%) und Fibronektin (39±2, 36±4 bzw. 36±2%) hatten keinen Einfluß auf die Restitution (n=6). Antilaminin führte zu einer signifikanten Zunahme der Schädigung auf 64±5% (p<0,05), die Histologie zeigte ein Fehlen der Defektdeckung. 100 µg/ml Laminin reduzierte die Mukosaschädigung auf 16±2% (p<0,05, n=6). Die Histologie zeigte, daß die Epitheldefekte nahezu vollständig von Enterozyten gedeckt waren. Luminale Säureexposition führte zu einem 50%igen R-Abfall (n=6). R war 3 h nach Schädigungsende 20% unter dem Ausgangswert. In der Antilaminingruppe war R nach 3 h 50% unter dem Ausgangswert, während R in der Laminingruppe den Ausgangswert nahezu erreichte (98% vom Vorschädigungswert; p<0.05 im Vergleich zur Kontrollgruppe, n=6). Alle anderen Testsubstanzen hatten keinen Einfluß auf den R-Verlauf.
Schlußfolgerungen: Histologie, Morphometrie und Elektrophysiologie zeigten, daß Laminin die Restitution des Kaninchenduodenums in vitro stimuliert. Daher glauben wir, daß die Wechselwirkung zwischen Epithelzelle und Basalmembran einen wichtigen Faktor zur Erhaltung der Epithelintegrität darstellt.
Summary
Background: Rapid restitution enables fast resealing of epithelial defects by migrating enterocytes. We investigated the effect of basal lamina proteins on restitution of rabbit duodenal mucosa after acidic damage.
Methods: Following 30 min of baseline incubation in Ussing chambers mucosal preparations of rabbit duodenum were injured by luminal exposure to 10 mM HCl for 10 min. Thereafter tissues were incubated for 3 h with buffer alone (controls) or luminal buffer containing testsubstances (n=6 per group). Resistance (R) was calculated from open circuit potential difference (PD) and short-circuit current (Isc). Damage was measured on hematoxylin and eosin stained sections and is given in % of mucosal surface.
Results: Acid exposure caused detachment of the upper 2 thirds of the villi resulting in short villus formation (54±4% damaged, n=8). Up to 3 h postinjury mucosal surface was partly resealed by flat enterocytes (36±2% damaged, n=6). Bovine serum albumin (38±5%), anticollagen IV (35±7%), antifibronectin (38±4%), 25, 50, 100 µg/ml collagen IV (34±3, 35±2, 33±4% respectively) and 25, 50, 100 µg/ml fibronectin (39±2, 36±4, 36±2% respectively) did not influence restitution (n=6). Damage was significantly increased by antilaminin (64±5%; p<0.05; n=6). Damage was significantly reduced by 100 µg/ml laminin (16±2; p<0.05; n=6). Laminin promoted enterocyte migration over the basal lamina. According to histology and morphometry laminin promoted electrophysiology to recover to baseline values, while R values further decreased in antilaminin exposed tissues when compared to controls (p<0.05). All other testsubstances did not have an effect on postinjury electrophysiology when compared to controls.
Conclusions: Our data show that laminin stimulates epithelial restitution of rabbit duodenum in vitro. Thus it is suggested that epithelial cell-basal lamina interactions are required for maintaining epithelial integrity.
