Zusammenfassung
An Hand eines Krankengutes von 87 Patienten mit Ulkusperforation wurden vor allem 2 Fragen beleuchtet:

Ist die primäre Übernähung eine infolge höherer postoperativer Letalität schlechtere Methode als die primäre Resektion?

2.
Ist der Patient nach Übernähung einer Sekundäroperation zuzuführen?

Zum 1.
Punkt konnte festgestellt werden, daß die höhere postoperative Letalität nach Übernähung in erster Linie auf Grund der in diesen Fällen wesentlich ungünstigeren Ausgangssituation zurückzuführen ist. Bezüglich

des 2.
Punktes wurden postoperative Magensekretionsanalysen herangezogen, die nach unseren Untersuchungen einen wesentlichen Aufschluß bezüglich der Prognose des Ulkusleidens geben können. Daneben konnte die unmittelbare postoperative Prognose an Hand von Verlaufskontrollen der Nierenfunktionsparameter (BUN undSerumkreatinin) aufgezeigt werden.
Summary
87 patients with perforated peptic ulcer were operated during the years 1962 to 1973. It was intended to answer 2 questions:

What therapy should be done: simple closure or primary resection?

2.
How long should be the interval for reoperation after simple closure.

The higher postoperative mortality after simple closure is caused by more high-risk patients in this group and both methods are practicable.

2.)
Our results have shown that postoperative gastric analysis is the most important examination for the indication of a second operation and the prognosis of the disease. In addition it could be shown that renal function, measured by BUN and blood-Kreatinin is a valuable factor of the prognosis during the early postoperative period.
