La prothèse de genou non contrainte HERMES

Abstract

The method of total knee replacement developed by Pr. J.M. Cloutier is based on the concept that both cruciate ligaments should be retained in order to allow physiological motion of the knee. The design of the three components of the HERMES non constrained total knee prosthesis duplicates normal anatomy and allows minimal bone resection. In case of absent anterior cruciate ligament, a special tibial plateau is available. The knee-joint distractor maintains the axial alignment of the limb as well as the adequate tension of ligaments during the procedure; it facilitates the exposure and the fixation of the implants with cement. Careful analysis of the results of 30 knees three years after operation shows good results except in four cases. Unsatisfactory scores are generally related to excessive medial release, and in one case to inadequate rotational alignment.

Soucieux de respecter au mieux l'anatomie et de réunir les meilleures conditions physiologiques, J.M. Cloutier a inventé la prothèse de genou HERMES composée d'un implant fémoral rigoureusement anatomique, d'une embase métallique en fer à cheval équipée de plateaux en P.E. pour un resurfaçage tibial conservant tout le pivot central, et d'un implant rotulien rond. Un implant tibial pour conservation du seul L.C.P. est disponible. Les implants sont scellés au ciment. Particulière par l'utilisation du distracteur, la technique opératoire conduit géométriquement à l'obtention d'un interligne perpendiculaire à l'axe mécanique du membre inférieur. Le matériel ancillaire est précis mais abondant, et la durée de l'intervention en pâtit. Une modeste série de 30 cas analysée avec sévérité illustre la méthode, avec des résultats dans l'ensemble satisfaisants. L'analyse des échecs ne met pas les implants en cause.
