Zusammenfassung
Die Reaktion des arteriellen Blutdruckes auf intravenöse Infusionen von Angiotensin II und Noradrenalin wird in Form von Dosis-Wirkungs-Beziehungen an 18 freiwilligen Versuchspersonen vergleichend untersucht. Dabei ist das Ausmaß der systolischen Blutdrucksteigerung der Dosis der betreffenden Substanz proportional, die Dosis-Wirkungs-Beziehungen nehmen einen linearen Verlauf. Noradrenalin steigert den systolischen Blutdruck um durchschnittlich 2,00±0,57 mm Hg/µg/min, Angiotensin um 9,8±2,8 mm Hg/µg/min. Letzteres wirkt demgemäß etwa 5mal stärker auf den systolischen Blutdruck als Noradrenalin. Der Wirkungsunterschied ist beim Vergleich der diastolischen Blutdruckänderungen (Angiotensin 11,0±2,34 mm Hg/µg/min; Noradrenalin 1,57±0,67 mm Hg/µg/min) mit einem Verhältnis von 7,0:1 höher, da diese Dosis-Wirkungs-Beziehungen beim Angiotensin steiler und beim Noradrenalin flacher (und gekrümmt) verlaufen als die systolischen. Die Pulsfrequenz nimmt unter den Versuchsbedingungen unter Noradrenalin um 16%, unter Angiotensin um 19% ab.
Blutmenge und Hämatokrit ändern sich unter Angiotensin praktisch nicht, während unter Noradrenalin innerhalb von 30 min eine signifikante Abnahme des Blutvolumens um durchschnittlich 7% und ein Anstieg des Hämatokrit um 8% auftritt. Als Ursache des unterschiedlichen Verhaltens beider Substanzen wird die stärkere venendrucksteigernde Eigenschaft des Noradrenalins angesehen.
Lactat, Pyruvat, der Lactat/Pyruvat-Quotient und das sog. „excess lactate“ ändern sich während der Zufuhr pressorischer Substanzen in therapeutischen Dosen nicht. Die Wasserstoffionenkonzentration des Venenblutes verschiebt sich unter Angiotensin zur alkalotischen Seite, möglicherweise steht dieser Befund in Zusammenhang mit einer durch die Angiotensin-Zufuhr eingetretenen Steigerung der endogenen Aldosteron-Sekretion.
Die Bedeutung der Befunde für die pressorische Schockbehandlung wird diskutiert.
Summary
To compare the effectiveness of i.v. angiotensin II and norepinephrine on arterial blood pressure dose-response-relations were investigated in 18 healthy volunteers. It was found that blood pressure elevation is directly proportional to the dose given in both drugs. The mean rise of systolic blood pressure was 2,0±0,57 mm Hg/µg/min with norepinephrine, and 9,8±2,8 mm Hg/µg/min with angiotensin II. Equal doses of angiotensin therefore cause a systolic blood pressure elevation 5 times greater than that of norepinephrine. Comparing the mean diastolic pressure elevations the ratio was 7:1 (angiotensin 11,0±2,34 mm Hg/µg/min; norepinephrine 1,57±0,67 mm Hg/µg/min). The heart rate in our series was slowed 16% with norepinephrine and 19% with angiotensin.
Blood volume and hematocrit show practically no change with angiotensin. With norepinephrine we observed a mean decrease of blood volume of 7% within 30 min and an 8% increase of the hematocrit. The cause of this difference we assume is the stronger effect of norepinephrine on the venous blood pressure.
The serum levels of lactate, pyruvate, lactate/pyruvate-quotient and the excess lactate did not change under the influence of therapeutic doses of pressure elevating drugs. There was a slight rise in venous blood pH with angiotensin possibly as a sign of increased aldosterone secretion.
The significance of our findings for the management of shock is discussed.
