287. Begrenzte Bedeutung des Intervalles als perioperativer Risikofaktor des perforierten Ulcus

Summary

In a retrospective study from 1946 to 1985, the interval between ulcer perforation and the beginning of surgical therapy was examined as a prognostic factor for perforated gastroduodenal ulcers. In recent years, the interval has increased, particularly in elderly patients and in women. Hospital mortality and the degree of peritonitis have increased with an interval. The prognostic significance, however, can be demonstrated only for patients aged 46–75 and for patients with diffuse peritonitis, but not when peritonitis is minimal or localized.

Zusammenfassung

In einer retrospektiven Studie der Jahre 1946–1985 wurde das Intervall zwischen Perforation und Behandlungsbeginn als wesentliches Merkmal des perforierten Gastroduodenalulcus untersucht. Das Intervall hat im Laufe der Jahre zugenommen, wobei ältere Patienten und Frauen ein besonders langes Intervall haben. Letalität und das Ausmass der Peritonitis nehmen mit der Länge des Intervalles zu. Prognostische Bedeutung hat das Intervall nur in der Altersgruppe 46–75 Jahre und bei diffuser, nicht aber bei minimaler oder lokaler Peritonitis.
