Dopo aver ricordato che le modificazioni del metabolismo glucidico, dapprima riconosciute solo attraverso la glicosuria, poi meglio individuate e studiate mediante la ricerca del tasso glicemico, rappresentano uno dei campi di indagine che fin dai secoli scorsi ha maggiormente attirata l'attenzione degli psichiatri, si sottolinea come tale particolare interesse sia oggi ancor più giustificato in base ad una serie di acquisizioni relativamente recenti, che vanno dal ruolo preminente dei carboidrati nel metabolismo della cellula nervosa all'efficacia della terapia insulinica nelle psicosi, e alla riconosciuta importanza del diencefalo, dell'ipofisi e delle ghiandole surrenali nei riguardi del ricambio glucidico e delle funzioni psichiche.

Da un punto di vista generale, tre sono le possibilità che, a seconda del diverso processo patogenetico, si devono prendere in considerazione nei rapporti traalterazioni glucidiche e disturbi mentali:

a)
Primitività del disordine metabolico cui consegue, in via più o meno diretta, un'alterazione psichica. Possono rientrare in questo gruppo, oltre ai disturbi che si hanno nelle ipoglicemie spontanee, parte di quelli che talvolta si osservano nel diabete (cosiddette “psicosi diabetiche”). Discutibile sembra invece al relatore tale meccanismo patogenetico per una particolare sindrome confusionale-onirica (oneirofrenia), che insorgerebbe secondoMeduna a causa di alterazioni del metabolismo glucidico dovute il piu spesso ad una parziale inattivazione enzimatica (inibizione dell'esochinasi).

b)
Primitività dell'alterazione psichica che si ripercuote secondariamente sul ricambio glucidico. Precisato che non si esamina l'influenza degli stress emotivi in soggetti psichicamente normali, dato che il problema psicosomatico del diabete è trattato in altra relazione, viene discusso se le modificazioni glicemiche che si hanno in individui normali per uno shock emotivo autorizzino ad ammetere (il che è assai dubbio) che anche i disturbi della glicoregolazione riconoscibili talvolta nello stadio acuto delle psicosi siano dovuti ad un'alterazione dello stato affettivo.

c)
Esistenza contemporanea di disturbi del metabolismo glucidico e dell'attività psichica come fenomeni tra loro indipendenti, ma esperessione di uno stesso processo morboso interessante contemporaneamente la sfera psichica e quella vegetativa. Tale processo, che può avere eziologia varia, è fondamentalmente rappresentato nella fase acuta da un turbamento dei rapporti funzionali tra lobo frontale e strutture basali, cui consegue da un lato il sovvertimento del normale ordine gerarchico tra le varie funzioni psichiche, dall'altro una diminuita capacità omeostatica del sistema nervoso vegetativo.

Nell'ambito clinico la condizione di cui in c) si verificherebbe soprattutto nelle psicosi acute a sintomatologia confusionali-onirica, nelle quali più frequentemente si riscontrano turbe del metabolismo glucidico. E' probabile tutta via che tale meccanismo patogenetico sia anche in gioco, seppure assieme ad altri fattori (quali il particolare orientamento endocrino-vegetativo, il tipo costituzionale, ecc.) in tutti i disturbi della glicoregolazione che insorgono nelle malattie mentali.

Viene poi esposta e discussa la nuova impostazione che è oggi possibile dare al problema dei rapporti tra alterazioni del ricambio glucidico e delle funzioni psichiche. Essa si fonda sul concetto che l'insorgenza e le caratteristiche della turba metabolica siano sempre dovute nelle psicosi all'intervento di più fattori (costituzionali e processuali), i quali possono essere non solo di intensità diversa tra loro, ma anche concordare o discordare rispetto alla direzione verso la quale tendono a spostare l'equilibrio del sistema glicoregolatore (aumentata o diminuita tolleranza agli zuccheri).

Infine si prende in considerazione il valore diagnostico e prognostico che riveste in psichiatria la presenza di alterazioni del ricambio glucidico, e in particolare vengono discusse le prove proposte daMeduna per distinguere la vera schizofrenia dalle forme pseudoschizofreniche, prove la cui validità, in base alle ricerche del Relatore, è risultata incostante.

Die anfangs nur durch die Glykosurie erkannten, später aber auch durch die Blutzuckerbestimmung besser untersuchten Veränderungen des Zuckerstoffwechsels stellen ein Forschungsgebiet dar, das schon seit Jahrhunderten die Aufmerksamkeit der Psychiater erweckt.
Nachdem heute dieses gesteigerte Interesse durch die relativ neuen Erkenntnisse über die besondere Stellung der Kohlenhydrate im Stoffwechsel der Nervenzelle, über die Wirksamkeit des Zwischenhirns, der Hypophyse sowie der Nebennieren in Bezug auf den Zuckerstoffwechsel und der psychischen Funktionen weitaus gerechtfertigter ist, werden die Beziehungen zwischen Stoffwechselveränderungen und geistigen Störungen geprüft und erörtert.

Vom allgemeinen Standpunkt aus ergeben sich, entsprechend einem verschiedenen pathogenetischen Grundmechanismus, drei Möglichkeiten dieser Beziehungen:

a)
Primitivität der metabolischen Störung, welcher auf direktem oder indirektem Wege eine psychische Veränderung folgt. In dieser Gruppe kann man, auβer den Beschwerden bei Hypoglykämie, jene, gelegentlich beim Diabetes (sogenannte “Diabetes-Psychosen”) auftretenden einschlieβen. Zweifelhaft erscheint dieser pathogenetische Mechanismus jedoch bei oneiroiden Zuständen (Oneirophrenia), die nachMeduna, durch die Veränderung des Zuckerstoffwechsels, öfters durch die partiale enzymatische Inaktivität hervorgerufen werden (Hemmung der Hexokinase).

b)
Primitivität der psychischen Störungen, die sich auf den Zuckerstoffwechsel sekundär auswirkt. In diesem Falle wird nicht nur die Beeinflussung des “Stress” bei psychisch normalen Individuen (das psychosomatische Problem des Diabetes), geprüft, sondern es wird auch erörtert, ob man, im Hinblick auf die durch einen starken emotionellen Schock bei Normalen auftretenden Blutzuckerveränderungen, die im akuten Stadium der Psychose manchmal zu beobachtenden Störungen des Kohlenhydrathaushaltes einer abnormen affektiven Spannung zuschreiben kann, was aber sehr zweifelhaft ist.

c)
Gleichzeitiges Vorhandensein von Störungen des Zuckerstoffwechsels und Störungen psychischer Art. Diese zwar untereinander unabhängigen Erscheinungen können jedoch Ausdruck eines gleichartigen krankhaften Prozesses sein, der die psychischen wie die vegetativen Funktionen gleichzeitig beinhaltet. Dieser Vorgang, der verschiedene Ursachen haben kann, stellt im akuten Stadium eine Störung der funktionellen Beziehung zwischen den Stirnlappen und der Hirnbasis dar. Daraus ergibt sich einerseits eine Störung der normalen hierarchischen Ordnung zwischen den verschiedenen Funktionen, anderseits eine verminderte homeostatische Fähigkeit des vegetativen Nervensystems.

Klinisch zeigt sich der Zustand von c) besonders bei akuten Psychosen mit onirischen Verwirrungszuständen, wobei man häufig Störungen des Zuckerstoffwechsels antrifft. Es ist jedoch wahrscheinlich, daβ sich dieser pathogenetische Vorgang, wenn auch im Verein mit anderen Faktoren (wie z. B. der konstitutionelle Typus, eine besondere, endokrin-vegetative Komponente usw.), in allen Regulations-störungen des Blutzuckerhaushaltes, im Rahmen einer Geisteskrankheit abspielt.

Darlegung einer neuen Auffassung, betreffend des Problem der Wechselbeziehungen zwischen Veränderungen des Zuckerstoffwechsels und der psychischen Funktionen. Diese Auffassung beruht auf dem Prinzip, daβ das Auftreten und die Kennzeichen der Stoffwechselstörungen in der Psychose immer von der Teilnahme verschiedener Faktoren konstitutioneller und prozessueller Art abhängen, diese zeigen nicht nur untereinander verschiedene Intensität, sondern können auch je nach der Richtung, in der sie das Gleichgewicht des Zuckerhaushaltes in Form verminderter oder erhöhter Zuckertoleranz verschieben, übereinstimmen oder differieren.

Zum Schluβ wird der diagnostische und prognostische Wert besprochen, der den Veränderungen des Zuckerstoffwechsels in der Psychiatrie zukommt. Auβerdem werden die vonMeduna vorgeschlagenen Methoden eingehend erörtert, welche zur Unterscheidung der eigentlichen Schizophrenie von den pseudo-schizophrenen Formen zu gelten hätten. Ihre Gültigkeit scheint jedoch auf Grund der Forschungen des Referenten nicht gesichert.

After having considered the fact that the modifications of the carbohydrate metabolism, which formerly were recognized only through glycosuria and were successively better distinguished and examined by means of researches made on the blood sugar, represent one of the fields of research that ever since the past centuries have most attracted the attention of psychiatrists, is stressed by the Reporter that this particular interest is to-day more justified on the basis of a series of relatively recent findings that range from the pre-eminent rôle of the carbohydrates in the metabolism of the nervous cells to the effectiveness of the insulin treatment and to the recognized importance of the diencephalus, the pituitary and adrenal glands with regard to the carbohydrate metabolism and psychic functions.

From a general point of view — necessarily schematic — there are three possibilities, which, according to the various pathogenetic processes, must be taken into account when considering the relationship between carbohydrate alternations and mental disorders.

a)
The primitiveness of the metabolic disorders, followed up, in a more or less direct way, by psychic alteration. Besides the disorders, which one meets with in the hypoglycemic crisis, those too that are sometimes observed in diabetes (the socalled “diabetes-psychosis”), may be included in this group. Doubtful it seems, on behalf of the Reporter this pathogenetic mechanism in the case of a particular dream-like confusional syndrome (oneirophrenia), that according toMeduna arise as a consequence of the alteration of the carbohydrate metabolism, due for the most part to partial inactivity of the enzymes (inhibition of the hexokinase).

b)
The primitiveness of the psychic alteration, having a secondary repercussion on the carbohydrate metabolism. We must note that the influence of the stress on normal subjects (the psychosomatic problem of diabetes) has not here been examined, but on the contrary the question if the blood sugar modifications that appear in normal subjects through emotive shock autorize us to admit (which is indeed doubtful) that the disorders in the glycoregulation to be seen sometimes in the acute phase of the psychosis are due to an alteration of the affective conditions.

c)
The contemporary presence of psychic and of the carbohydrate metabolism disorders as being phenomena independent one of the other but being both expressions of the same morbose condition contemporarily affecting the psychic and the vegetative functions. This process that may have a different etiology is fundamentally represented in the acute phase by a disorder in the functional relations between the frontal lobe and the basal structures causing, on one side, an upsetting of the normal hierarchic order between the various psychic functions and on the other, a diminished homoestatic capacity of the autonomic nervous system.

Clinically the condition of c) takes place above all in acute psychosis with oneiric-confusional symptomatology, in which more frequently disorders in the carbohydrate metabolism are met with. It is, however, probable that such a pathogenetic mechanism is to be found too, though together with other factors (such as the particular endocrino-vegetative condition, the constitutionaly type etc.), in all disorders of the glycoregulation that arise in mental diseases.

The new impostation that it is now possible to give to the problem of the relation between the alterations of the carbohydrate metabolism and psychic functions is then explained and discussed. It is based on the concept that metabolic disorders arise and are characterized always in psychosis by some factors (constitutional and processual) which not only may be different one from other as to their intensity, but also either agree or disagree as to the direction in which the equilibrium of the glycoregulatory system is deviated (increased or diminished tolerance for sugar).

Finally the diagnostic and prognostic value of alteration of the carbohydrate metabolism in psychiatry is discussed, and in particular the tests suggested byMeduna for distinguishing genuine schizophrenia from schizophreniform states, the validity of which, on the basis of the researches of Reporter, were not always constant.

Après avoir rappelé que les altérations du métabolisme des glucides, reconnues d'abord seulement à travers la glycosurie et mieux étudiées et individualisées ensuite par la recherche du taux de la glycémie, représentent un des champs d'étude qui, dès les siècles passés, a particulièrement attiré l'attention des psychiatres, on remarque que ce particulier intérêt c'est aujourdhui encore plus justifié d'après une série d'acquisitions relativement récentes qui vont du rôle prééminent des hydrates de carbone dans le métabolisme de la cellule nerveuse, jusqu'à l'efficacité du traitement avec l'insuline et à la bien reconnue importance du diencéphale, de l'hypophyse et des glandes surrénales à l'égard du métabolisme des glucides et des fonctions psychiques.

Du point de vue général et suivant le différent processus pathogénique trois possibilités sont à considérer en ce qui concerne les rapports des altérations des glucides et des troubles mentaux:

a)
Désordre primitif du métabolism, qui est suivi, plus ou moins directement, d'une altération psychique. Dans ce groupe peuvent être inclus outre que les troubles que l'on remarque dans les crises hypoglycémiques, ceux qu'on observe quelquefois dans le diabète (≪psychoses diabétiques≫). Ce mécanisme pathogénétique semble au contraire discutable au rélateur pour un particulier syndrome confuso-onirique qui s'insurgerait selonMeduna, à cause d'altérations du métabolisme des glucides, dues à une partielle inactivité des enzymes (inhibition de l'hexokinase).

b)
Altération psychique primitive qui se répercute secondairement sur le métabolisme des glucides. On doit préciser quel'on ne prend pas en examen l'influence des stress émotifs chez les sujets psychiquement normaux parce que le problème psycho-somatique du diabète est traité dans un autre rapport, mais on discute si les altérations de la glycémie qu'un shock émotif peut produire dans un individu normal nous autorisent à affirmer (ce qui est bien douteux) que les troubles de la régulation des glucides qu'on remarque quelquefois dans le stade aigu des psychoses sont aussi produits par une altération de l'état affectif.

c)
Existence contemporaine des troubles du métabolisme des glucides et psychiques, comme des phénomènes indépendants entre eux, mais expression du même état morbide qui intéresse en même temps l'équilibre psychique et le végétatif. Cet état, qui peut avoir une différente étiologie se manifeste dans le stade aïgu par un trouble des relations fonctionnelles entre le lobe frontal et les structures basales, ensuite par un bouleversement de l'ordre hiérarchique entre les différents fonctions psychiques d'un côté, et de l'autre par la diminution de la capacité oméostatique du S. N. V. centrale.

Dans le domaine clinique la condition c) semble se réaliser surtout dans les psychoses aiguës à symptomatologie confuso-onirique dans lesquelles on aperçoit plus fréquemment des troubles du métabolisme des glucides. Il est probable toutefois que ce mécanisme pathogénique joue un rôle bien qu'avec d'autres facteurs (caractère constitutionnel, particulier équilibre endocrino-végétatif, etc.) dans tous les troubles de la régulation des glucides qui se manifestent dans les maladies mentales.

On expose et on discute ensuite la nouvelle interprétation qu'il est possible donner aujourd'hui au problème des relations entre les altérations du métabolisme glucidique et des fonctions psychiques. Elle se base sur la conception que la présence et les caractéristiques des troubles métaboliques soient toujours causées dans les psychoses par plusieurs facters (constitutionnels et du processus), qui non seulement peuvent être entre eux d'une intensité différente, mais aussi être concordes ou discordes par rapport à la direction vers lequelle ils tendent à déplacer l'équilibre de la régulation des glucides (tolérance pour les glucides augmentée ou bien diminuée).

On considère enfin la valeur diagnostique et pronostique d'une métabolisme glucidique altérée dans les psychoses aiguës, et en particulier on discute les épreuves proposées parMeduna pour distinguer la véritables schizophrénie des formes de pseudoschizophrénie, épreuves dont la validité, d'après les recherches du relateur, est resultée incostante.
