Zusammenfassung
Versuche an isoliert durchströmten Meerschweinchenherzen, in denen unter strenger Trennung toxisch und nichttoxisch wirkender Cardenoliddosen die Kaliumbilanz aus der arteriovenösen Kaliumdifferenz ermittelt wurde, haben zu folgenden Ergebnissen geführt:

k-Strophanthin, Digitoxin und Digitoxigenin bewirken schon in nichttoxischen, positiv inotrop wirkenden Dosierungen eine signifikante Kaliumabgabe. Dabei besteht eine deutliche Beziehung zwischen der Stärke des positiv inotropen Effektes und der freigesetzten Kaliummenge.

Der maximale Kaliumverlust unter der Einwirkung positiv inotrop wirkender Cardenolidkonzentrationen beträgt nur etwa 3% des im Herzen vorhandenen Kalium; er läßt sich deshalb durch Methoden, die mit einer stärkeren Streuung belastet sind (z.B. K-Analysen im Herzmuskel selbst) nicht nachweisen.

Toxische Cardenoliddosen, die Frequenzanstieg, Kontraktur oder Arrhythmie verursachen, bewirken eine wesentlich stärkere Kaliumabgabe des Herzmuskels und in der Dosiswirkungskurve eine Änderung des Regressionskoeffizienten; das spricht dafür, daß dem Kaliumverlust im Bereich toxischer und nichttoxischer Glykosiddosen verschiedene Mechanismen zugrunde liegen.
An isolierten Meerschweinchenvorhöfen findet bei 10 min langer Inkubation in einer nicht toxischen Digitoxigeninlösung (3 · 10−7) ebenfalls eine signifikante Kaliumabgabe statt.
Summary
Toxic and nontoxic amounts of digitalis compounds had the following effects on the potassium outflow from the isolated perfused guinea pig heart:

Nontoxic amounts of k-strophanthin, digitoxin and digitoxigenin with positive inotropic effects caused a significant rise in the potassium concentration of the perfusate. There was a significant correlation of the magnitude of the inotropic effect and the amount of liberated potassium.

The maximum loss of potassium caused by these nontoxic amounts of digitalis compounds was only 3% of the potassium of the heart muscle. Therefore, it is not demonstrable by less sensitive methods, such as potassium assay of heart tissue.

Toxic amounts of digitalis compounds, which led to a rise in rate and contractility or produced arrhytmias, caused a much larger loss of potassium and had a different regression coefficient of the dose-response curve. This indicates that the loss of potassium following toxic and nontoxic doses of digitalis compounds is based on different mechanisms.
In the isolated guinea pig atrium, 10-minute incubation with a nontoxic amount of digitoxigenin (0,3 μg/ml) led to a significant release of potassium.
